To open an account with VOS Selections you must complete the New Accounts Application. This form can be downloaded by clicking on the link on this page or we can fax it to your place of business. If you would like the form faxed to you, please call (212) 967-6948.

Please note, the following information will be required of you when opening a new account:

• A valid license issued from the New York State Liquor Authority or the New Jersey Alcoholic Beverage Control
• A completed ST-120 form (download blank ST-120 form)
• A copy of the Certificate of Authority.
